About the Role

As a Home Health RN, you will be responsible for delivering one-on-one skilled nursing care, performing comprehensive assessments, developing individualized care plans, monitoring patient progress, and providing education and support to patients and families. You will work under the direction of the Director of Professional Services to help patients regain independence and improve their quality of life while avoiding unnecessary hospital visits.

About You

Required:

Completion of an accredited nursing program.
Active Registered Nurse license in the state of North Carolina.
At least 1 year of clinical nursing experience.

Preferred:

Home Health experience.
A strong sense of independence and a commitment to patient-centered care.
Reliable transportation and valid driver’s license.
