About the Role
About You
- Graduate of an accredited nursing program (Associate or BSN preferred).
- Active RN license in the state, current CPR certification, and valid driver’s license.
- 1–2 years of acute care experience; home health care experience preferred.
- Strong communication, critical thinking, and clinical documentation skills.
- Capable of lifting, positioning, and transferring patients.
- Experience in home health care settings.
- Ability to coordinate care as a Case Manager.
