About the Role
As a Remote Chronic Care Management LVN, you will engage with patients through telehealth check-ins and care coordination to ensure high-quality support. You will work closely with Registered Nurses (RNs) and Medical Directors to deliver seamless patient experiences while adhering to Medicare standards. Responsibilities include ongoing care coordination, conducting check-ins, supporting RNs with documentation, tracking activities for billing compliance, and facilitating communication among healthcare providers.
About You
Required:
- Active, unrestricted LVN license in California.
- Strong communication, empathy, and interpersonal skills.
- Proficiency with electronic medical records (EMRs) and digital health platforms.
- Highly organized and proactive in managing multiple patient caseloads.
Preferred:
- Experience with Medicare PCM/CCM or similar chronic care programs.
- Background in caring for older adults or neurodegenerative disease populations.
- Familiarity with telehealth or hybrid care delivery.
- Exposure to quality improvement or care coordination initiatives.
