As a Resident Services Coordinator, you will be responsible for managing the Department and collaborating with a dynamic care team. Your role will include ensuring the best quality of life for residents, coordinating care services, and supporting families in navigating the care process.
About You
Required:
- Master’s degree in social work (MSW) is required.
- Goal of obtaining the New York State Certification in Social Work (LMSW).
Preferred:
- Experience in geriatrics and discharge planning with a minimum of two years’ experience.
- Training in assessing case management and providing excellent customer services to the aging population.
