About the Role
As an RN case manager, you will make a difference every day by performing patient assessments, collaborating with the care team to develop and implement care plans, and teaching patients and families appropriate care techniques. You will also be responsible for maintaining accurate patient care records, communicating regularly with the clinical manager, and performing on-call responsibilities as assigned.
About You
Required:
- One year of nursing experience.
- Current RN license specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
