About the Role
About You
- Registered Nurse, Nurse Practitioner, or Clinical Nurse Specialist with a minimum of 2 years’ experience in health care and/or case management.
- Active Michigan Registered Nurse (RN) license with no disciplinary action.
- Must reside in the state of Michigan.
- Ability to travel to homes and community settings for face-to-face assessments.
- Experience working with the adult population, disease management.
- Knowledge of community health and social service agencies.
- Exceptional communication and interpersonal skills.
- Ability to work with minimal supervision.
- Ability to use electronic medical records and various software applications.
- Valid driver's license, car insurance, and access to an automobile.
- Must have a separate room for a home office and high-speed internet access.
- BSN.
- Experience with in-home assessment and care coordination.
- Experience with health promotion, coaching, and wellness.
- Experience with Medicaid Long Term Care.
- Previous managed care experience.
- Bilingual in Spanish, Arabic, or Chaldean Neo-Aramaic.
