About the Role
As an RN - Case Manager, you will be responsible for assessment, planning, facilitation, and advocacy to promote optimal, cost-effective outcomes for individuals. You will work Monday to Friday from 8:00 a.m. to 5:00 p.m. EST, coordinating services and communicating effectively with various stakeholders.
About You
Required:
- Minimum 3 years of clinical practice experience in hospital, home health, ambulatory care, or other alternative care settings.
- Current, unrestricted state licensure as an RN.
Preferred:
- Experience in healthcare and/or managed care industry.
- Case Management Certification (CCM).
- Case management experience.
Benefits
- Competitive pay rate of $40.72/hr. on W2.
- Potential for contract extension beyond 3 months.
