About the Role
As a Registered Nurse (Home Health), you will be responsible for delivering high-quality nursing care to patients in their homes. Your key responsibilities will include assessing patient needs, developing care plans, and coordinating with healthcare teams to ensure optimal patient outcomes. You will work Monday to Friday during the day shift, with additional on-call responsibilities and weekend coverage as needed.
About You
Required:
- 1+ year of professional experience as an RN (Home Health experience preferred).
- Active RN License in the State of IL.
- Associates Degree (or higher) in Nursing.
- BLS and CPR Certification.
- Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance.
Preferred:
- Please provide your RN License Number.
