Summary

About the Role
As a Care Manager at Village Care, you will be responsible for care planning, coordination, and health assessments. You will monitor service quality, track patient progress, oversee transitions, and collaborate with the Interdisciplinary Care team to implement interventions. This position allows you to make a significant impact on patient well-being while working in a customer-centric environment.
About You
Required:
- Valid NYS RN License.
- Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings.
- Associate's degree or higher.
Preferred:
- Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole.
- Strong problem-solving abilities and exceptional communication skills.
