About the Role
As an RN Admissions Nurse – Hospice (PRN), you will conduct clinical evaluations, meet with patients and families to explain hospice services, and collaborate with clinical teams to ensure timely onboarding of hospice services. You will also provide emotional support, maintain documentation for compliance, and support the clinical team with visits as needed.
About You
Required:
- Graduate of an accredited nursing program; Associate's or Bachelor's degree preferred.
- Current RN license in the state of practice or valid multistate compact license.
- Minimum of one (1) year of hospice, palliative care, home health, or hospital-based nursing experience preferred.
- Strong clinical assessment, communication, critical thinking, and documentation skills.
- Valid driver's license and reliable transportation required.
Preferred:
- Familiarity with hospice eligibility requirements, services, and Medicare Conditions of Participation.
- Ability to compassionately educate and guide patients and families through emotionally sensitive conversations.
