About the Role
As an RN Care Manager, you will manage patient flow by collaborating with nursing and interdisciplinary teams, coordinating care plans, conducting patient stay reviews, developing discharge plans, and ensuring compliance with regulations. You will also lead daily huddles and participate in multi-disciplinary meetings to address barriers to patient care.
About You
Required:
- Bachelor's of Science in Nursing required.
- Current licensure to practice nursing in the State of Illinois.
- Two years of experience in an acute care setting required.
- Previous leadership experience required.
- Case management certification required or obtained within one year of eligibility.
Preferred:
- General knowledge of case management processes including discharge planning, health care reimbursement, utilization review process, and applicable community resources.
