About the Role
As an RN Case Manager, you will be responsible for performing clinical assessments, managing care for high-risk members, and developing individualized care plans. You will collaborate with healthcare teams and supervise non-RN clinicians, ensuring members receive appropriate services and support. This field-based role requires travel to client sites and occasional office attendance.
About You
Required:
- Current, unrestricted RN license in applicable state(s).
- Minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
Preferred:
- MA/MS in Health/Nursing.
- Experience working with older adults in care management or similar capacity.
- Experience managing a community and/or facility-based care management case load.
