About the Role
As a Hospice RN Case Manager, you will be responsible for utilizing the nursing process to assess, plan, implement, and evaluate patient care. You will collaborate with interdisciplinary teams, provide direct care, and educate patients and families on personalized care plans. The position requires a compassionate approach to ensure the comfort and quality of life for patients.
About You
Required:
- Current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
Preferred:
- Experience in hospice care.
- Proficiency in electronic medical records (EMR) systems.
