About the Role
As an RN Case Manager, you will perform patient assessments, supervise LPNs and HHAs, and promote patient health through teaching and rehabilitative measures. You will also be responsible for timely documentation, communicating patient progress, and participating in clinical development programs.
About You
Required:
- One (1+) year of clinical experience as a Registered Nurse (RN).
- Current RN license specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Preferred:
- Experience in home health care.
- Ability to work independently and manage time effectively.
