About the Role

As an RN Field Case Manager, you will be responsible for administering skilled nursing care on an intermittent basis, making initial evaluations, coordinating services, and counseling patients and their families. You will also prepare clinical notes, supervise other nursing personnel, and participate in in-service programs and performance improvement activities.

About You

Required:

Graduate of an approved school of professional nursing and currently licensed in the state of Michigan.
Two (2) years nursing experience.

Preferred:

Experience with Kinnser - EMR.
Home Healthcare experience.
Experience in ICU, PICC Line, Wound Care, and Infusion.
