RN Hospice Nurse Case Manager

AMEDISYS HOME HEALTH is seeking an RN Hospice Nurse Case Manager to join their team in Washington, PA. The role involves providing patient assessments and collaborating with the care team to develop and implement care plans for hospice patients.

As an RN Hospice Nurse Case Manager, you will perform patient assessments, make referrals to other hospice care disciplines, maintain individualized patient care progress records, and teach patients and families appropriate care techniques. You will also communicate regularly with the clinical manager and care team, perform on-call responsibilities, and complete other assigned duties.
