About the Role
As an RN, Inpatient Case Manager, you will be responsible for collaborating with providers, clinical teams, health plans, and external partners to advocate for patients and caregivers. Your daily tasks will include conducting comprehensive assessments, coordinating clinical care, and educating patients and families about their healthcare options. You will also play a key role in managing readmissions and patient satisfaction.
About You
Required:
- Bachelor’s Degree in Nursing or equivalent experience.
- 3 years in a clinical nursing role.
- Current State of PA RN licensure or multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
- Professional Certification within 5 years of start date.
- Critical thinking and problem-solving skills.
- Strong communication and collaboration skills.
Preferred:
- Nationally recognized Case Management Certification.
- Knowledge of motivational interviewing techniques.
- BSN.
