About You
Required:
- Active Registered Nurse (RN) License and/or Licensed Social Worker (LSW/LISW) in the state of Ohio.
- Minimum two years of recent clinical experience in direct care of elders or adults with disabilities.
- Experience in case management, care planning, and assessment in a healthcare setting.
Preferred:
- Ability to work in a fast-paced environment meeting department goals and KPIs.
- Tech savvy with proficiency in common software and web-based applications.
