About the Role
As a Registered Nurse, you will be responsible for delivering high-quality clinical services, coordinating care with other healthcare professionals, completing nursing assessments, ensuring patient eligibility for services, and developing individualized care plans. You will work to maintain continuity of patient care by collaborating with physicians, agency personnel, patients, families, and community resources.
About You
Required:
- Current and unrestricted RN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR Certification or ability to complete within 90 days of hire.
- Demonstrated ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Preferred:
- 1+ years of Home Health experience.
- Demonstrated ability to work independently.
- Proven solid communication, writing, and organizational skills.
