About the Role
As an RN Registered Nurse Hospice, you will be responsible for conducting comprehensive assessments of patients and families, developing and revising individualized Plans of Care, providing professional nursing care, coordinating with the Interdisciplinary Group (IDG), and ensuring continuity of care. You will also supervise ancillary personnel, maintain clinical documentation, and participate in performance improvement initiatives.
About You
Required:
- Education sufficient to meet certification, licensure, or registration requirements.
- Active state RN license.
Preferred:
- 1 year of recent experience in hospice, palliative care, home health, acute care, and/or skilled nursing as a registered nurse.
- Working knowledge with computer applications, i.e. Microsoft Word/e-mail.
