The Home for Little Wanderers is seeking a Senior Mental Health Intensive Care Coordinator to join their team in Somerville, MA. The role involves overseeing and delivering Individual Care Coordination services to youth with serious emotional disturbances.
About the Role
As a Senior Intensive Care Coordinator, you will be responsible for performing initial assessments, developing and monitoring Individual Care Plans, acting as a liaison with various agencies, and ensuring the delivery of necessary services to families. You will also supervise direct reports, participate in management activities, and maintain documentation to meet quality assurance standards.
About You
Required:
- Master’s degree in Psychology, Social Work, Mental Health, Counseling, Family Therapy, or a related field.
- At least three (3) years of experience in providing outpatient behavioral health services to youth and families.
- Supervisory experience and experience working collaboratively with state agencies and consumer advocacy groups.
- Certification in the Massachusetts CANS.
Preferred:
- Experience with home-based Wraparound models.
- Knowledge about the communities served.
- Fluency in a second language such as Spanish, Portuguese, or Haitian Creole.
- Computer literate including Microsoft Office and Internet Explorer.
