About the Role
As a Service Navigator, you will play a crucial role in helping clients understand and access the array of services offered by LOGAN. Your responsibilities will include developing individualized service plans, facilitating communication between clients and service teams, and ensuring seamless service delivery that supports vocational, social, and integrated outcomes. You will also assess client satisfaction and modify service plans as needed, while promoting a culture centered around an employment-first mindset.
About You
Required:
- A minimum of five years’ experience with individuals with intellectual and developmental disabilities.
- Demonstrated ability to communicate effectively with clients, coworkers, and supervisors.
Preferred:
- Bachelor’s degree in social services, Education, or a related area.
- Experience in developing and implementing engaging, creative learning curriculum.
