Oakwood Village is seeking a Licensed Practical Nurse (LPN) for their Skilled Nursing Facility in Madison, WI. This part-time/per diem role involves contributing to the assessment, planning, implementation, and evaluation of resident care.
About the Role
As a Licensed Practical Nurse, you will collaborate with nurses, interdisciplinary staff, and physicians to manage resident care. Your responsibilities will include direct resident and family care, mentorship of CNA staff, and working closely with registered nurses. You will also document and report changes in resident conditions, update care plans, and ensure compliance with nursing regulations.
About You
Required:
- Licensed practical nurse license required.
- CPR Certified required.
Preferred:
- One year relevant nursing experience in long term care preferred.
- Knowledge of HSS 132 and all Federal tags preferred.
- Knowledge of Medicare A requirements preferred.
