About the Role
As a Social Services Assistant, you will assist the Social Service Director/Consultant in various tasks including planning, developing, and evaluating social service programs. You will collaborate with administration and medical staff, participate in community planning, conduct resident assessments, and provide emotional support to residents and their families. Additionally, you will refer residents to appropriate agencies and assist with discharge planning and solutions for social and practical problems.
About You
Required:
- Must possess a high school diploma or equivalent.
Preferred:
- 6 months experience in a Social Services Assistant position in a skilled nursing facility.
