About the Role
As a Social Services Designee, you will be responsible for overseeing the social services function in the facility, collaborating with the interdisciplinary team, managing behavior and mental health care, and ensuring compliance with federal and state regulations. You will also participate in discharge planning, maintain communication with residents' families, and manage departmental budgets.
About You
Required:
- Social Services Certification (ACSW) from an accredited program within 3 months of hiring.
- Ability to collaborate effectively with interdisciplinary teams.
Preferred:
- Previous experience in a skilled nursing facility (SNF).
- Full Covid-19 vaccination up to date.
