About the Role
As a Social Services Specialist, you will work under the direction of the Director of Social Services, conducting interviews with residents and families to develop psychosocial histories and care plans. You will assist with legal status determinations, inform residents and families of their rights, and ensure residents receive necessary evaluations and appointments. Additionally, you will provide support in discharge planning and work closely with various stakeholders to address resident needs and concerns.
About You
Required:
- High school diploma or equivalent required.
- Ability to interact effectively and professionally with residents, families, and staff.
Preferred:
- Previous experience with geriatric population or in long-term care setting.
- AA or Bachelor's degree.
