About the Role
As a Hospice Social Worker, you will assess the psychosocial status of patients and families, conduct social evaluations, provide counseling, and coordinate resources. You will collaborate with an interdisciplinary team to develop individualized care plans and maintain accurate documentation of services provided.
About You
Required:
- Master's degree in Social Work (MSW).
- Ability to work tactfully with patients, families, and the community.
Preferred:
- One year of social work experience in a hospice setting.
- Strong understanding of hospice philosophy and end-of-life issues.
