Summary

About the Role
As a Social Worker, you will telephonically assess and identify needs, coordinate services, provide education and counseling, and establish relationships with community agencies. You will also participate in discharge planning and care coordination meetings, monitor client progress, and contribute to enhancing healthcare delivery.
About You
Required:
- Licensed Social Worker (LMSW OR LCSW) is required.
- Demonstrated clinical experience and previous experience working with the Medicaid population.
- 2-3 years of managing long term care or long-term support services experience required.
- Uniform Assessment System (UAS) or Person-Centered Service Planning (PCSP) experience required.
- Excellent oral and written communication skills.
- Excellent computer skills including strong working knowledge of Word and Windows based applications.
- Ability to work well in a dynamic, deadline-oriented environment.
- Experience in situations demanding strict adherence to confidentiality.
- Ability to solve problems while exhibiting clinical judgment.
Preferred:
- Knowledge of member centric, motivational interviewing strategies, and strength-based models.
