- Master’s degree in Social Work
- Current Rhode Island LICSW
- 2+ years of prior healthcare / behavioral health experience
- Strong problem-solving and organizational skills
- Ability to communicate effectively in writing and verbally
- Geriatric care experience
- Proficient in Microsoft Office Suite and EMR systems
- Ability to manage patient complexity and multiple clients with diverse needs
