About the Role
As a Social Worker, you will be responsible for developing comprehensive social histories and assessments for residents, coordinating admissions and assessments, assisting families with transitions, participating in care plan development, ensuring compliance with regulations, maintaining confidentiality, and coordinating discharges and transfers.
About You
Required:
- Bachelor's degree in Social Work, Gerontology, or a related field.
- Must be a licensed Social Worker in the current state of residence.
- At least 2 years of experience in a health care setting, preferably Long Term Care.
Preferred:
- Excellent oral, written, and interpersonal skills.
- Ability to present a professional manner and handle stressful situations.
