About the Role
As a SOUTH Brooklyn Hospice RN Case Manager, you will work onsite at a leading hospital, responsible for assessing patient cases, collaborating with clinical teams, and facilitating hospice admissions. You will serve as a key intermediary to ensure clear communication and coordination of care, conduct thorough assessments, and provide education to enhance patient and family understanding of hospice services.
About You
Required:
- Registered Nurse (RN) license in New York State.
- Experience in hospice or palliative care.
Preferred:
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team environment.
