Founder and owner Forrest L. Preston opened the first facility in 1970 and incorporated the business in 1976. Headquartered in Cleveland, Tennessee, today Life Care employs more than 38,000 associates and operates more than 200 skilled nursing, post-acute, Alzheimer’s, and senior living campuses in 27 states.
