Trinity Health is seeking a Substance Abuse Program Coordinator - Days - Full Time LISW to oversee and manage all aspects of the Substance Abuse Program at Diley Ridge Medical Center, ensuring effective, efficient, and quality services.
About the Role
The Substance Abuse Program Coordinator is responsible for assessment, planning, directing, controlling, and evaluating the Substance Abuse Program. This includes managing financial, human, quality, cost, and planning strategies; standardizing service delivery; collaborating with nursing, inpatient physicians, and community agencies; providing education to leadership; managing budgets; participating in direct patient care assessments; promoting service excellence and safety culture; leading performance improvement initiatives; facilitating interdisciplinary care model development; and maintaining visibility and accessibility to patients and staff.
About You
Required:
- MSW and certification as a clinical social worker
- Current LISW license to practice as a social worker in the State of Ohio
- 2-3 years of experience in discharge planning or crisis intervention in an acute care or ambulatory setting
- Strong organizational skills and ability to adapt to changing regulatory requirements
- Excellent clinical assessment and analytical skills
- Effective interpersonal, critical thinking, decision-making, and conflict management skills
- Ability to work independently with minimal supervision and exercise sound judgment
- Computer competency
- Ability to manage multiple assignments accurately and efficiently
- Demonstrated ability to collaborate with physicians and multidisciplinary teams
Preferred:
- Case Management Certification by ACMA or CMSA
- Chemical Dependency certification or ACM/CCM
- Knowledge of utilization management, discharge planning, care coordination, transitions of care, and third-party payers
