About the Role
As a Supportive Housing Case Manager, you will conduct intake assessments, develop individualized housing stability/service plans, and assist clients in securing permanent housing. You will coordinate various supportive services, advocate for clients, and facilitate crisis intervention when necessary. This position requires collaboration with multiple stakeholders and a commitment to using harm-reduction and trauma-informed approaches.
About You
Required:
- Bachelor’s degree in social work, psychology, human services, or related field.
- At least 1-2 years of case management experience with people experiencing homelessness, mental illness, substance use, or trauma.
- Proficiency with Microsoft Office.
- Excellent organizational, documentation, and time-management skills.
- Ability to work independently in the field and manage a flexible schedule.
Preferred:
- Knowledge of Housing First, harm reduction, trauma-informed care, and motivational interviewing.
