Summary
Project Renewal is seeking a Team Lead, Social Work to join their Support and Connection Center team in Rochester, New York Metropolitan Area. The role involves overseeing a team providing treatment services for adults with mental health and/or substance use issues.
About the Role
As a Clinical Team Lead, you will conduct rapid assessments, engage guests in wellness and discharge planning, and facilitate linkages to community-based services. You will provide direct care, collaborate with the on-site team for coordinated care, and promote dignity-based care. Additionally, you will model best practices, counsel guests, and ensure accurate data entry into case management systems while promoting the professional development of your team.
About You
Required:
- LMSW, LCSW, LMHC, LP or master’s level counselor with CASAC.
- A minimum of four (4) years of experience working with homeless and criminal justice involved individuals with either a diagnosis of mental illness or a substance use disorder.
- Strong communication and problem-solving skills.
Preferred:
- Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
- Proficiency in case management software, electronic health records, and Microsoft Office Suite.
Benefits
- Salary range of $70,000 - $75,000.
- Full-time employment with a structured work schedule from Sunday to Thursday, 3-11 PM.
