Summary
North Shore Community Health is seeking a Telehealth Community Health Worker - Hybrid to join their team in Salem, MA. This part-time role focuses on assisting patients in accessing telehealth services and providing culturally-appropriate education.
About the Role
As a Telehealth Community Health Worker, you will work directly with patients to enhance their understanding and usage of telehealth services. Your responsibilities will include providing digital literacy education, assisting new patients with paperwork, and managing communications between clinical staff and patients. You will also help transition in-person appointments to telehealth and utilize EMR tools to identify barriers to digital equity.
About You
Required:
- Experience in a community health center setting or related experience in health/human services.
- Demonstrated strong communication skills with individuals and/or groups.
- Strong analytical, problem-solving, and planning skills.
- Ability to prioritize multiple projects and work independently.
Preferred:
- BA in health or human services.
- Spanish or Portuguese language skills.
- Experience working with patients with chronic behavioral health needs.
