About the Role
As a Transitional Care Navigator, you will serve as a clinical consultant and educator, reviewing hospital documentation, engaging with patients and providers, and collaborating with care teams to improve patient outcomes. You will also identify trends for earlier hospice engagement and maintain workflows for patient assessments and education planning.
About You
Required:
- Bachelor’s degree in marketing, business administration, or related fields preferred.
- Licensed as a registered nurse in the state of practice preferred.
- Minimum of two years relevant experience.
- Valid driver’s license and proof of insurance required.
Preferred:
- Experience in business development or sales, particularly in healthcare.
- Strong communication, strategic thinking, and problem-solving skills.
