About the Role
As a Case Manager, you will be responsible for establishing relationships with youth, conducting client intake and assessments, creating individualized treatment plans, initiating crisis intervention, collaborating with an interdisciplinary team, and facilitating life skills activities. You will also transport youth to appointments, model positive behaviors, maintain records, and engage with volunteers and partners.
About You
Required:
- Bachelor's degree in Social Work, Human Services, or related field or equivalent experience required.
- Minimum two (2) years' experience preferred.
Preferred:
- Master's degree preferred.
- Experience in mental health, trauma, substance abuse, and homelessness.
