About the Role
As a Youth Services Care Coordinator, you will be responsible for providing case management services to a caseload of 12-13 clients, conducting intake assessments, creating and implementing treatment plans, facilitating team meetings, and developing crisis/safety plans. You will collaborate with various service providers and transport clients as needed, ensuring that families receive the necessary support tailored to their individual needs.
About You
Required:
- Bachelor's degree in sociology, social work, psychology, or a closely related field.
- Reliable transportation, a valid driver's license, and automobile insurance.
Preferred:
- Master's degree in a relevant field.
- Experience in case management or working with youth in mental health settings.
