About the Role
As a Step Down Navigator, you will be responsible for identifying and meeting with eligible patients, ensuring all aspects of the step down process are followed, assisting with discharge planning, maintaining electronic data collection, and providing reports to senior leadership. You will also help identify trends to support program expansion and improve treatment accessibility.
About You
Required:
- Strong communication skills to effectively engage with patients and staff.
- Experience in behavioral health or related field.
- Ability to collect and analyze data.
- Familiarity with insurance verification processes.
- Knowledge of treatment planning and discharge processes.
- High School Diploma.
Preferred:
- Experience in a hospital or healthcare setting.
- Familiarity with electronic health records (EHR) systems.
- Previous experience in patient navigation or care coordination.
