About the Role
As a Family Partner, you will coordinate with providers to support families in achieving their Care Plan goals. Your responsibilities include engaging parents in activities, assisting caregivers, maintaining policies for peer support services, and participating in Wraparound Care planning. You will also be responsible for completing paperwork, attending meetings, and providing transportation when necessary.
About You
Required:
Experience as a caregiver of a youth with special needs, preferably with mental health needs.
High school diploma or GED with a minimum of two years of experience with the target population or navigating child and family serving systems.
Preferred:
Bachelor's degree in a human services field from an accredited university and one year of experience working with the target population.
An associate's degree in a human service field from an accredited school and one year of experience working with children/adolescents/transition age youth.
