Summary
GREEN MEADOWS HOME HEALTH CARE INC is seeking a Home Health COTA Full Time to join their team in Fountain Valley, CA. The role involves providing skilled therapy services to patients in their homes to restore function and improve mobility.
About the Role
As a Home Health COTA, you will evaluate patients' functional status and therapy needs, develop individualized treatment plans, implement therapy interventions, educate patients and caregivers, document clinical notes, and collaborate with the interdisciplinary care team to ensure comprehensive patient care.
About You
Required:
- Graduate of an accredited Occupational Therapy program.
- Current Occupational Therapy license in the state of practice.
- CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong organizational, communication, and documentation skills.
- Ability to work independently in a home setting.
Preferred:
- Minimum of one year of OT experience preferred (home health experience is a plus).
- Experience with electronic health records (EHR).
- Familiarity with Medicare/Medicaid and home health documentation standards (e.g., OASIS).
- Bilingual abilities a plus.
